What this website collects, why it collects it, and how long it is kept.

## Who is responsible

Mr Mohamed Mohyudin is the data controller for personal information submitted through this website, registered with the Information Commissioner's Office under reference **ZB525269**. Enquiries about this notice may be sent to [mnmohyudin@doctors.org.uk](mailto:mnmohyudin@doctors.org.uk).

## What is collected

Very little, and only when you choose to send it. This site sets no cookies of its own and runs no analytics, advertising or tracking scripts.

-   **If you subscribe to Notes**, the form on the Notes page collects your email address. Alongside it we record the date you subscribed and the country your connection came from, as reported by our hosting provider.
-   **If you email** the address published on this site, we hold your message and whatever contact details it contains.
-   **When you visit any page**, our hosting provider, Cloudflare, processes technical data such as your IP address and browser type in order to deliver the page and protect the site from abuse. We do not use that data to identify or profile you.

Please do not send clinical details or anything about your health to the email address on this site. Clinical enquiries belong with the practice, which has a confidential route for them.

## Why it is used, and on what basis

Your subscription address is used only to email you when a new note is published — a handful of times a year at most. The lawful basis is your consent, which you can withdraw at any time using the unsubscribe link in every email or by writing to the address above.

Emails are used only to answer you. The lawful basis is our legitimate interest in responding to correspondence sent to us.

## Who it is shared with

Subscription addresses are passed straight to the mailing-list service that sends the notes, which processes them on our instructions and for no other purpose. Cloudflare hosts the site and handles traffic to it as described above. Nothing is sold, and nothing is shared with advertisers.

## How long it is kept

A subscription is kept until you unsubscribe. Correspondence that does not lead to a consultation is deleted within twelve months. Where an enquiry becomes part of a clinical record, it is retained under the retention schedule for private medical records rather than this notice.

## Your rights

Under UK data protection law you may request a copy of the information held about you, ask for it to be corrected or erased, object to its processing, or ask that processing be restricted. Write to the address above and you will receive a response within one month.

If you are not satisfied with the response, you may complain to the Information Commissioner's Office at [ico.org.uk](https://ico.org.uk/).

## Security

The site is served over HTTPS only. Subscription addresses are forwarded directly to the mailing-list service and are not stored on the web infrastructure.
