Who is responsible
Mr Mohamed Mohyudin is the data controller for personal information submitted through this website, registered with the Information Commissioner's Office under reference ZB525269. Enquiries about this notice may be sent to mnmohyudin@doctors.org.uk.
What is collected
Nothing. This site hosts no forms, sets no advertising or analytics cookies, and does not track you across other sites. If you write to the email address published here, that correspondence is held only for as long as it takes to answer you.
Please do not include detailed clinical information in the enquiry form. The secretary will arrange a confidential route for that discussion.
Why it is collected
To respond to your enquiry and, where relevant, to arrange a consultation. The lawful basis is your consent, which you give by ticking the consent box, and which you may withdraw at any time by writing to the address above.
Who it is shared with
Enquiries are read by the practice secretary and by Mr Mohamed Mohyudin. They are transmitted through the practice's hosting and mail providers, who process the data on the practice's instructions and for no other purpose. Nothing is sold, and nothing is shared with advertisers.
How long it is kept
Enquiries that do not lead to a consultation are deleted within twelve months. Where an enquiry becomes part of your clinical record, it is retained under the retention schedule applying to private medical records rather than this notice.
Your rights
Under UK data protection law you may request a copy of the information held about you, ask for it to be corrected or erased, object to its processing, or ask that processing be restricted. Write to the address above and the practice will respond within one month.
If you are not satisfied with the response, you may complain to the Information Commissioner's Office at ico.org.uk.
Security
The site is served over HTTPS only. Enquiry submissions are passed directly to the practice's mail routing and are not stored on the web infrastructure.
